5. Add 10% for Shipping, Handling and Insurance - (Exceptions – Free Shipping on Orders Over $250)
6. Add 6% Sales Tax for orders placed within Kentucky
7. Contact Information –Include Name*, Address*, Shipping Address (if different), Phone Number* & E-mail Address (*required)
8. Payment Information – We accept Check, Money Orders, and listed Credit Cards (Master Card, Visa and Discover with credit card number, expiration date and name given on card)
